Here is the form to help you get started with the Honors Program:

Here is the form to submit to the Coordinator upon finishing the entire Program:

Submitting Forms

• Each semester, the student should complete the Honors Contract with the instructor that is willing to offer an honors addendum to the syllabus. The course addendum and additional course workload must approved by the Coordinator. 
• Forms may be submitted to the Coordinator by either the faculty or student, as long as all signatures are present and packet is completed. 
• All completed forms must be submitted by the deadline.
