The following forms require you to log into your GCCCD single sign on account through Self-Service, Outlook, Canvas, etc.

• Grade and Status Change Form
• Reinstatement Form
• Add - Drop Form

The Following forms require Adobe Reader to view.

• Add-Drop Card
• Authorization for Level or Section Change
• Excessive Absence Drop Card
• Incomplete Grade Contract
