SLPA Requirement Updates
All forms will be kept in the Students SLPA file
in the SLPA Office.
1. Background Check and Drug Screen
Process
This process is to be done yearly and can be
accessed via the web. Directions are on the New Forms link on
the SLPA Home Page. You can also access it be going to the web
link
www.grossmontbackgroundcheck.com this starts the process.
Print out the email that has Background Search Confirmation
and Receipt for (Your Name) on the subject line and bring
into the SLPA office for your Lab Slip. This email will come
from
Creditorder@americandatabank.com. Once the Lab is
done it will take 48 hrs to get the results back to you via email
again. Keep this email and review it to see if there are no
flags. If it is completed you are go to good to clinical.
If there is a flagged notice on the finial notice you will need to
review the finial and correct what you can either off the background
search or the drug screen (if diluted only). This school and
facilities we use are a zero tolerance and drug free work areas.
2. Standard Physical Requirements
This form is to be signed off yearly and can be
accessed via the web. This is to verify that the student is
able to do the physical requirements needed in the classroom and/or
clinical settings. If there are limitations the student will
need to bring in a doctors note to verify the limitation and then
when the limitation is lifted we will need a doctors release and a
newly signed standard physical requirement page. This form can
be accessed by the web on the New Forms link.
3. Physical Form
This form is to be done yearly to conform with
our college and contract policies. This form needs to be
signed off by a physician and have a business card or stamp attached
to it. This form can be accessed by the web on the New Forms
link.
4. Dental Form
This form is to be done first year to conform
with our college policies. This form needs to be signed off by
a Dentist and have a business card or stamp attached to it.

5. Consent Form
This form is to be signed off yearly and can be
accessed via the web. This is to give the SLPA Program consent
to release your health information to the clinical facilities that
you will be going to. This form can be accessed by the web on
the New Forms link.
6. Immunization Form
This form is to be completed when you enter into
the program. All your immunizations need to be checked off by
the Health Services at Grossmont College. If your doctor does
your form please have them stamp or have a business card attached.
If you do not have records you can always have a titer done. A
titer is blood lab work which will show if you are positive or
negative to the specific immunization. This form can be
accessed by the web on the New Forms link.
7. TB/PPD (2-Step) Form
This form is to be completed when you enter into
the program. You can get a TB/PPD test here at the Health
Services Office or your physician. If your doctor does your
form please have them stamp or have a business card attached.

8. CPR Resources
This form is to be completed when you enter into
the program. A CPR Card is required to go out to clinical.
The SLPA Program will only
accept the Healthcare Provider Card. It is a
2 year card and will last the whole 2 years you are in the program.

9. Malpractice Insurance
This form is to be completed when you enter into
the program. HPSO is our carrier who will provide you with the
insurance coverage that you need. You can always contact them
directly by web www.hpso.com or by
phone 1-800-982-9491. Your start date should always be the
first day of class of the first year.
10. Workers Compensation - Clinical
This form is to be completed only if you have had
an injury at the clinical site. You will download the pages
and fill them out and fax them to us. Priority is to let your
on-site supervisor know and fill out any paperwork they have, then
do the College requirements. Call the SLPA office let them
know that you had an injury and call the college clinical
supervisor. This paperwork goes to the College Benefits
Coordinator, Wendy Corbin.
11. Flu Vaccine Consent
This form is to be completed when you enter into
the program. You can get a Flu Vaccine here at the Health
Services Office or your physician's office. If your doctor
does your form please have them stamp or have a business card
attached. This form can be accessed by the web on the New
Forms link.
NOTE:
At any time there can be either
an update or a change to any form, consent due to the health care
industry needs. We as a college have to conform to the
standards of the community since we are sending out students.
